You hereby consent to receive treatment or services from Sheneka Dalmage Nurse Practitioner in Family Health, PLLC d/b/a Touchd Medical Spa upon receiving the disclaimer and notice related to the service/treatment you are scheduled to receive.The information provided to you by the Company is not suited or intended to be a replacement for professional medical advice or treatment. Always seek the advice of your physician or medical professional with respect to any services received by the Company, medical condition or questions you may have regarding the treatment/service. There is no replacement for personal medical treatment and advice from your personal physician regarding any treatment or any the impact of receiving the treatment with any medical condition you may have.   

If you have any questions or concerns regarding a treatment that you are scheduled to receive, you should obtain medical clearance from your personal medical physician prior to receiving such services. The Company cannot tell you whether you can or cannot receive a specific treatment or service. This is only determinable by your medical provider.  

You, as the client or customer, are responsible to disclose any and all allergies, medical conditions, medical history, or concerns that you may have prior to booking treatment or service. The Company cannot be held liable for any adverse reactions or unsatisfactory outcomes to any treatments, products, or services received. The Company cannot be liable if you fail to disclose whether you are pregnant. Any pregnant women or customers should disclose that they are pregnant PRIOR to any bookings or scheduling of services. The Company cannot tell you whether you can or cannot receive a specific treatment or service while pregnant. This is only determinable by your medical provider and you should receive medical clearance prior to contacting, scheduling or booking a treatment or session with the Company. However, we will provide you with information provided to our Company by third-parties regarding what is considered “safe or otherwise” for pregnant women. If you are unsure, we recommend not obtaining a service to error on the side of caution.

Touchd Medical Spa Policies

Touchd Medical Spa Scheduling, Deposit & Cancellation Policy

Scheduling Deposit: A 50% non-refundable deposit is required to reserve all appointments, as appointments block significant time on the providers schedule and limit our ability to reserve other clients during that period. For questions prior to booking please call our office at (914) 278-9029. Deposits are applied toward the total cost of the service at checkout and are collected automatically through our booking/payment system.

Refund Policy: All sales are final. There are no refunds for any services, treatments, packages, deposits, memberships, or products, including services that are unused or partially used.

Appointment Cancellation & Rescheduling Policy: A minimum of 24 hours notice is required to cancel or reschedule any appointment, cancellations or rescheduling made 24 hours or more in advance, your deposit may be transferred one time to a future appointment or different service within 30 days of the original appointment date. Cancellations or rescheduling made less than 24 hours in advance, your deposit is forfeited and a new deposit is required to rebook.

Neurotoxin Touch-up Policy: One complimentary neurotoxin touch-up may be offered at the providers discretion and must be scheduled and completed within 14 days of the original appointment date. Touch-up requests after 14 days are not eligible for the complimentary service. Touch-up appointments require at least 24 hours notice to cancel or reschedule. Cancellations or rescheduling made with less than 24 hours notice will result in forfeiture of the complimentary touch-up.

No-Shows: The deposit is forfeited and a new deposit is required to rebook.

Late Arrivals: Arriving more than 15 minutes late may result in a shortened appointment time to maintain the schedule. If we are unable to proceed due to lateness, the appointment may be canceled and the deposit will be forfeited. Appointment time cannot be extended due to late arrival as this may affect the experience of other clients. 

Packages/Multi-session Services: This policy applies to each scheduled session within a package.

Payment & Credit Card Authorization: A valid credit or debit card is required to remain on file to reserve appointments. By scheduling, you authorize Touchd Medical Spa to charge the card on file for deposits, late cancellations, no-shows, and any remaining balances due. Card transactions may be subject to standard processing fees charged by the payment processor. Payments are processed through a secure third-party vendor. Touchd Medical Spa is not responsible for interruptions or security incidents originating within third-party systems.

Training + Events Policy

To provide a seamless experience for every guest and participant, all TOUCHD events and training are reserved in advance and subject to the policies below.

Events

A nonrefundable deposit is required to secure your event date. Your reservation is not confirmed until the deposit has been received.

Final guest count and any selected enhancements must be confirmed prior to the event date. Pricing is based on the confirmed number of guests and package selected.

Cancellations or requests to reschedule must be submitted at least 14 days prior to the event. Deposits may be transferred one time to a new available date when adequate notice is provided. Cancellations made within 14 days of the event may result in forfeiture of the deposit and any additional payments made.

Late arrivals may reduce the length of your event or individual services so that we can remain on schedule.

All medical aesthetic or wellness services offered as party enhancements are available to eligible adult guests only and may require consultation, medical history review, consent, and clinical clearance prior to treatment.

Outside food, décor, vendors, or special arrangements must be approved by TOUCHD in advance.

TOUCHD Academy 

Enrollment is confirmed once registration and required payment have been completed.

Due to the small group and hands-on nature of TOUCHD Academy training, registration fees and deposits are nonrefundable.

If you are unable to attend, your registration may be transferred one time to another available training date when written notice is provided at least 7 days in advance.

Cancellations or no shows made within 7 days of the scheduled training are not eligible for transfer or refund.

Participants are expected to arrive on time. Late arrival may affect participation in hands on portions of the course and does not guarantee additional instruction time.

Certain courses may require an active professional license, certification, or proof of eligibility. Participants are responsible for ensuring they meet all stated prerequisites before registering.

Completion of a TOUCHD Academy training does not guarantee employment, licensure, certification eligibility, or independent authority to perform a service outside the participant's professional scope of practice.

General Policy

TOUCHD Medical Spa and TOUCHD Academy reserve the right to modify, reschedule, or cancel an event or training due to unforeseen circumstances, safety concerns, staffing needs, weather, or insufficient enrollment. If TOUCHD cancels a scheduled event or training, participants will be offered an alternate date or appropriate credit.

By reserving an event or registering for training, guests and participants acknowledge and agree to the applicable policies.
